Documents Needed For Tax Preparation

These are some documents you will want to collect, prior to your appointment.  Please provide as many applicable documents as possible.

Individual Client Documents to Bring:

Please bring the following documents with you to your appointment:

For all appointments:

​

  • Copies of any notices received from IRS or Franchise Tax Board (or other state agency)

​

If I will be preparing your tax return(s):

​

  • Tax organizer (see blank organizer & video on my website on how to complete the organizer ).  Completing the organizer is not required for our first meeting.

  • Source documents such as W-2, Forms 1099, 1098's, etc.

  • Complete questionnaire.

  • Amounts & dates of any estimated or extension tax payments made.

If you have already filed your most recent tax return:

  • Year to date pay stub(s)

  • Other current year, year to date information/estimates

​

Business Client Documents to Bring:

​

Please bring the following documents with you to your appointment:

  • Client intake form

  • Copies of past two years federal and state(s) income tax returns

  • The following payroll tax returns:

    • Quarterly Forms 941​ & DE 9

    • Annual Forms W-3 & 940

    • W-2(s) for shareholders & officers (if applicable)

  • Backup copy of QuickBooks and password for QuickBooks file (if applicable).  If not using QuickBooks, please provide:

    • Balance Sheet for the tax year ended.

    • Profit and Loss (aka Income Statement) for the tax year ended.

    • General ledger for the tax year ended (ideally in PDF or Excel format).

  • Source documents such as Forms 1099-MISC & 1099-K 

  • Copies of year end bank, business credit card and loan statements showing year end balances.  With the credit card statements be sure it covers the year end (it will most likely have some of the following month's activity on it).

  • Copies of current year's Forms 1099 that your business issued to independent contractors and landlords (required when paying over $600 for services or rents - see my Form 1099 page for more information).

  • Copies of any notices received from IRS or Franchise Tax Board (or other state agency)

  • Consent form (if you have previously worked with a tax professional).  This is not required but can be helpful.
